I, the parent or legal guardian of the participant, give permission for my child to attend and participate in all activities at Andre Drummond’s All-Star Basketball Camp. I acknowledge that participation in basketball and related activities involves an inherent risk of injury.
In the event of a medical emergency, I authorize camp staff to secure necessary medical treatment for my child. I understand that I am responsible for any medical expenses incurred.
I understand that despite precautions, exposure to infectious diseases including COVID-19 is possible. I acknowledge the risk and agree not to hold Andre Drummond, the camp, staff, or volunteers liable for any exposure or illness.
I hereby release and hold harmless Andre Drummond, camp staff, sponsors, and volunteers from any claims arising from my child’s participation in camp activities.
By signing this form, I agree not to pursue legal action against Andre Drummond, his affiliates, sponsors, staff, volunteers, or any facility associated with the camp for any injury, illness, loss, or damages that may arise from participation. All activities are undertaken at my child’s (or my) own risk.
The camp reserves the right to dismiss any camper if necessary. Grounds for dismissal include:
I give permission for Andre Drummond’s All-Star Basketball Camp to photograph and video my child during camp activities. These may be used in promotional materials, social media, or media coverage.
